Preparing for Your First Appointment

A little preparation can help make your first appointment go smoothly. We ask new patients to arrive approximately 15 minutes before their scheduled appointment so there is enough time to complete any necessary paperwork and get checked in.

Please bring the following with you:

• Photo Identification – Bring a valid form of identification for our patient records.

• Dental Insurance Information – If you have dental insurance, bring your current insurance card and any necessary policy information.

• Previous Dental Records – Records from your previous dental provider can give our team helpful information about your dental history.

• Recent Dental X-Rays – If available, please arrange to have recent X-rays transferred to our office or bring copies with you.

• Relevant Medical Information – Bring information about current medications, health conditions, allergies, or other medical history that may affect your dental care.

• Completed Patient Forms – Completing the appropriate forms before your appointment can help make the check-in process faster.

Dental Insurance & Financial Information

Wasilla Dental Center accepts most major dental insurance plans. Because insurance benefits and coverage vary significantly between plans, we recommend contacting our office if you have questions about your specific insurance before your appointment.

Please note that Wasilla Dental Center does not accept Medicaid, Denali KidCare, or Medicare.

Our team can help you better understand the information available regarding your dental benefits, but patients are ultimately responsible for understanding their individual coverage and any portion of treatment not covered by insurance.
